This Test Security/Confidentiality Agreement should be used by staff in accordance with the Usability, Accessibility, and Accommodations Guidelines and submitted to a District Coordinator.  It may be used for scribes and test readers.

This exemption form is required for schools/school districts to request an exemption from the state mandated ELA, Math, and Science assessments. This form is not required for out-of-state, homeschooled, international, or visiting students from another state.

An overview of the Braille options in the 2018-2019 Smarter Balanced administration.

The Smarter Balanced Usability, Accessibility, and Accommodations Guidelines will shape the delivery of online testing for all students, including those with visual, auditory, linguistic, or physical needs.  These guidelines were developed by the Smarter Balanced Consortium in collaboration with member states and nationally recognized experts on English language learners and students with disabilities.

The purpose of this guide is to provide support for the Smarter Balanced Assessment Consortium states in their efforts to communicate information to districts and schools for the successful implementation of the Smarter Balanced assessments.

Single digit (1-9) multiplication table is a non-embedded accommodation for Grade 4 math items and above.   This is to be used only for students with a documented and persistent calculation disability (i.e., dyscalculia).  This can be printed for students requiring this accommodation.

A paper-based table listing numbers from 1–100 is a non-embedded accommodation for Grade 4 math items and above. Students with visual processing or spatial perception needs may find this beneficial, as documented in their IEP or 504 plan. This can be printed for students requiring this accommodation.
